The Umbrella Scheme of Atal Vayo Abhyuday Yojana(AVYAY), being run by the Department of Social Justice and Empowerment, includes components to provide financial security, healthcare, nutrition, shelter, welfare etc. for senior citizens. Under one of such components, namely, the Integrated Programme for Senior Citizens(IPSrC), Grant-in-Aid is given to Implementing Agencies for running and maintenance of Senior Citizens Homes where basic amenities like shelter, food, medical care, entertainment opportunities etc. are provided free of cost to indigent senior citizens. Under RashtriyaVayoshri Yojana(RVY), assisted living devices are distributed free of cost, in camp mode, to senior citizens belonging to the families living below the poverty line or having Rs. 15000/- as monthly income and suffering from age related disabilities. The Elderline: National Helpline for Senior Citizens(NHSC) (Toll-free No. 14567) provides free information, guidance, emotional support and field intervention in cases of abuse and rescues in order to improve the quality of life of senior citizens. The component namely Senior-care Ageing Growth Engine(SAGE) aims to encourage youth to think about the problems of the elderly and come out with innovative ideas for the elderly care and promoting them into start-ups by providing equity support.

Under Indira Gandhi National Old Age Pension Scheme(IGNOAPS) of the National Social Assistance Programme(NSAP), a fully funded Centrally Sponsored Scheme of the Department of Rural Development, Government of India, monthly pension at the rate of Rs.200/- per month per beneficiary to elderly persons in the age group of 60-79 years belonging to Below Poverty Line(BPL) households, is being paid. The rate of pension is increased to Rs.500/- per month per beneficiary on reaching the age of 80 years. The States/Union Territories are encouraged to provide top up amounts of at least an equivalent amount to the assistance provided by the Central Government so that the beneficiaries could get a decent level of assistance. At present, the States/Union Territories are adding Top up amounts ranging from Rs.50/- to Rs.3000/- per month per beneficiary under the IGNOAPS of NSAP. The assistance under NSAP pension schemes is sanctioned up to the scheme-wise, State/Union Territory-wise cap of beneficiaries under the scheme. At present, the number of beneficiaries under the IGNOAPS in the country is around 2.21 crore and the Scheme has achieved almost 100% saturation in all States/Union Territories. The States/Union Territories have the option to provide pension from their own sources in case there are more eligible beneficiaries over and above the State/Union Territory cap under the NSAP pension schemes.

The Ministry of Health and Family Welfare launched the National Programme for Health Care of the Elderly(NPHCE) in 2010-11 with a view to provide dedicated healthcare services to senior citizens at various level of State Health Care delivery system i.e., at Primary, Secondary and Tertiary health care, including outreach services. The Programme has two components, namely National Health Mission(NHM) i.e., Primary and Secondary care service delivery through District Hospitals(DH), Community Health Centres(CHC), Primary Health Centres(PHC), Sub-Centre/Health & Wellness Centres, and Tertiary Component i.e., these services are being provided though Regional Geriatric Centres(RGCs) located at 19 Medical colleges in 18 states of India and two National Centres of Aging(NCAs) one in AIIMS, Ansari Nagar, New Delhi and another in Madras Medical College, Chennai. It also includes Research on health issues pertaining to senior citizens. Further, Government launched Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana

(PMJAY) to cover 10 crore poor and vulnerable families (approx. 50 crore beneficiaries) providing coverage of up to Rs.5 lakh per family per year for secondary and tertiary hospitalization. With the launch of the Ayushman Bharat-PMJAY, the RashtriyaSwasthya Bima Yojana(RSBY) and the Senior Citizen Health Insurance Scheme(SCHIS) have been subsumed in it. All enrolled beneficiary families of RSBY and SCHIS are entitled for benefits under the Ayushman Bharat-PMJAY.

INDIAN PHARMACEUTICAL INDUSTRIES IN THE TIMES OF COVID-19 PANDEMIC

The covid pandemic has actually benefitted the pharmaceutical industry and helped in the growth and development of this industry. The Indian pharmaceutical industry is the worldโ€™s third largest drug producer by volume and the countryโ€™s market manufactures 60 percent of vaccines globally[1]. This constitutes 40 to 70 percent of the supply to satisfy the World Health Organizationโ€™s (WHO) demand for Diphtheria, Tetanus, and Pertussis (DPT) and Bacillus Calmette Guerin (BCG) vaccines and 90 percent of the global demand for the measles vaccine[2]. In this covid pandemic, the Indian pharmaceutical industry has played a vital role in distributing affordable and low-cost generic drugs to millions of people around the globe.

GROWTH AND DEVELOPMENT OF INDIAN PHARMACEUTICAL INDUSTRIES

The covid-19 pandemic presented several opportunities for the pharmaceutical companies and helped them to think differently. It helped them to act in a sense of urgency for all the patients who were looking for cheap and affordable medicines. In this pandemic, Indian companies have risen and developed in the field of therapeutics by re-purposing the dugs.

This pandemic also gave the pharmaceutical companies opportunities to work in collaborations with some major global companies for the purpose of developing the covid vaccine in turn enhancing the global connections. Serum Institute of India partnered with Oxford University, Zydus Cadila partnered with Gamaleya Institute of Russia, Panacea Biotec collaborated with US-based Refana Inc and there are many other companies who collaborated for the purpose of producing good and effective vaccines against the deadly virus. These collaborations and partnerships helped the companies to make effective vaccines, develop a stand in therapeutics, and also reach the global market.

Indian pharma industry took a proactive approach during the pandemic and also became successful in proving its mettle in complex and specialty generics. These companies also ensured regular manufacturing by not hindering manufacturing even for a single day. With the development in the pharma industry, eCommerce platforms and industries also developed. In the current pandemic, a great deal of motivation was given to E-pharmacy which helped in ensuring that patients received their medication despite lockdown. The domestic pharma market turnover in India has reached Rs. 1.4 lakh crores (equivalent to $ 20.03 billion) in 2019 as per the government data[3]. This is actually an increase from Rs. 1.29 lakh crores in 2018[4].

CHALLENGES FACED BY INDIAN PHARMACY INDUSTRIES DUE TO PANDEMIC

The source of APIs plays a very crucial role in the strategic plan of the Indian pharmaceutical industry to combat covid-19. A major challenge faced by this industry is the supply of APIs. Active Pharmaceutical Ingredients (API) are defined as the active ingredients contained in a medicine. This pandemic has highlighted the dependence of the Indian pharma industry on imported APIs. Today, 60 percent of Indiaโ€™s API requirement is imported[5]. In commonly used APIs, such as cephalosporins, azithromycin, and penicillin, the dependence is as high as 90 percent [6]. ย Of the total imports of APIs and intermediates into India, China accounts for 65โ€“70 percent [7]. This is a problem faced by almost the whole west depends upon china for their API supplies. This pandemic has also highlighted low healthcare coverage in India. n terms of healthcare spending, India has one of the lowest healthcare budgets with just 1.26 percent of GDP being spent on healthcare[8]. India ranks 155th out of the 167 countries in terms of hospital bed availability (as per the Human Development Report 2020) with just five beds available for every 10,000 Indians[9]. ย Such a sudden rise in covid cases in India highlighted the shortage of hospital beds, medicines, and availability of laboratory tests. As pandemics have always shifted the way people react. A similar change has been seen in the consumption pattern of various consumers. There has been an increase in the case of online ordering and e-consultations, especially in the case of chronic diseases. These challenges can result in long-term impacts on this industry.

STRATEGIES FOR PROMOTING INDIAN PHARMA PRODUCTION

Initiatives like Production Linked Incentives (PLI) schemes for bulk drugs and medical devices introduced by the government for the industryโ€™s self-reliance have given a major boost to this industry. These incentives are to the tune of INR6,940 crore and INR3,420 crore, respectively, and encompass greenfield projects for bulk drugs and intermediates, and the establishment of three bulk drug parks[10]. In addition to this, the government should take some steps in removing the financial and technical barriers prevailing within this industry. This will in turn help in reducing the dependency of the Indian pharma industry on china for APIs.

The pharmaceutical companies have now identified the importance of backward integration which is expected to bring greater reliability, improve the quality of production, reduce dependence on external sources of supply, and help in increasing the efficacy of manufacturing. Several key representatives from the pharmaceutical industry and NITI Aayog have suggested fostering the approvals of pharmaceutical infrastructure developments, clearance from the environment ministry, and providing tax exemptions and subsidies for the development and promotion of the pharmaceutical industry hubs could benefit the market[11].

Over the last few years, many tourists have been visiting India for their medical treatment. The government has also relaxed rules for the tourists by issuing rapid airport clearances and fast-track medical visas, thus helping in the promotion of this industry.

CONCLUSION

This covid pandemic has been a boon to the Indian pharmaceutical industry. Medicine spending in India is expected to grow between 9-12 percent over the next five years, leading India to become one of the top 10 countries in terms of medical spending[12]. It is now important for all pharmaceutical companies to shift their product portfolio toward chronic diseases like covid drugs, antidepressants, anti-diabetes, cancers, etc. which are on the hike nowadays. Many initiatives have been taken up by the government to reduce healthcare expenses and make it affordable for the whole population of the country. The introduction of generic drugs has also paved the way for the benefit of pharma companies. It is now important to focus on the rural healthcare system and provide the rural areas with the necessary drugs and preventive vaccines. Doing so will help a lot in the development of pharma companies in India and will also lead to the development of the country as a whole.

PLIGHT OF WOMEN IN AFGHANISTAN

The Afghan women, maybe more than anybody else, have dreaded the Taliban’s return. There have been many advances in women’s rights over the last 20 years, which appear to be set to erase nearly overnight.

The Taliban, a political and military force, is said to have started in Islamic schools in Northern Pakistan in the early 1990s. Its aim was to restore order in Afghanistan following the withdrawal of Soviet troops in 1989, as well as to impose a harsh form of Sharia law. By 1998, the organisation had seized 90% of Afghanistan’s territory.

Once in control, the organisation garnered worldwide condemnation for a slew of human-rights violations. The ban on female education above the age of ten as well as harsh limitations on day-to-day liberties, were among the stringent mores imposed on women and its influence has frequently threatened to expand beyond, to places like Pakistan, where the organisation memorably shot teenager Malala Yousafzai in 2012. It destabilised several regions of the nation due to battles with US and UK forces on a regular basis, and Afghan people were continued to be assaulted. Many would agree that the political and cultural status of Afghan women had improved significantly since the Taliban’s collapse in late 2001.

The Bush administration’s acceptance of women’s rights and empowerment as rationale for its assault on the Taliban is long gone. In February 2020, US-Taliban peace talks were concluded, with the US pledging a quiet departure in exchange for an end to hostilities. Afghan leaders and top military generals have warned that the government will collapse without foreign assistance. It looks like the worst has transpired only weeks before Biden’s deadline of September 11th.

The Taliban rule wreaked havoc on the institutions and the economy, which had already been ravaged by decades of conflict and the Soviet scorched-earth counterinsurgency policy.

As a result, 3.5 million Afghan females were enrolled in education, with 100,000 of them enrolled in academic institutions. Women’s life expectancy increased from 56 to 66 years in 2017 and maternal mortality fell from 1,100 per 100,000 live births in 2000 to 396 per 100,000 in 2015. By 2020, women made up 21% of Afghan public workers, including 16% of top management positions, and 27% of Afghan parliamentarians.

 These benefits for women have been dispersed inequitably, with women in metropolitan areas benefiting considerably more than women in rural regions. Despite formal legal empowerment, life for many rural women has not improved much since the Taliban era, notably in Pashtun regions but also among other rural minority groups. Many Afghan males are staunch conservatives. Families often let their daughters to complete a primary or secondary education before proceeding with planned marriages. The burqa is worn by the majority of Afghan women in rural regions without any pushing from the Taliban.

What is the situation for women in Afghanistan now?

Women’s rights in Afghan had arguably maintained pace with many other Western countries prior to the 1970s. Women were granted the right to vote in 1919, one year after women in the United Kingdom. In the 1950s, gender segregation was eliminated, and in the 1960s, a constitution was enacted that included women in political life. As the region became more unstable in the 1970s, these rights were steadily eroded.

Only 38% of the international humanitarian response plan for Afghanistan is financed as of August 2021. This gap might result in the loss of specialised protection services for 1.2 million children, putting them at risk of abuse, recruitment, child labour, early and forced marriages, and sex abuse. About 1.4 million females, many of whom are survivors of domestic abuse, would be left without access to safe spaces where they may receive full care.

Females, who have experienced life with rights and freedoms, are among the most exposed as a result of the Taliban’s fast progress in Afghanistan. As the Taliban capture control of Kabul, they risk losing their hard-won achievements.

While they worked alongside male colleagues and in communities that were unfamiliar with people in positions of power to help establish a truly democratic civil society, they also wanted to pave the way for future generations to follow in their footsteps.

The Taliban offers itself a broad range of possibilities by claiming that they will โ€œprotectโ€ women’s rights under sharia but refusing to explain how women’s rights and life in Afghanistan will alter if they achieve their goals. Even if the government did not openly adopt as cruel a system for women as in the 1990s, the Taliban’s dispositions are quite likely to undermine women’s rights, impose cultural prohibitions on women, and reduce socio-economic possibilities for them.

In summary, even with this change in behaviour, the Taliban in power would almost certainly strive to curtail Afghan women’s legal rights, exacerbating their social, economic, and political circumstances. How much and in what manner, is the question.

Learning Disability (LD)

A child with a learning disability finds difficulty in reading, listening, speaking, understanding mathematical concepts, and with general comprehension. It is a neurological condition that affects the brain’s ability for sending, receiving, and processing information. It is important to make a note that LD is not a cause of physical or mental illness, economic or cultural background. Neither indicates that the child is weak or lazy. Some children that begin with slow learners but cope with their studies in the future don’t fall under LD. Children’s approach to a specific type of learning is not LD but a child’s interest.ย 

Causes of Learning Disability

There is no specific cause of LD. However, there are certain factors that can lead to learning disabilities.

  1. Heredity: It is seen that the child with parents who had LD is likely to grow the same as well. 
  2. Illness during pregnancy or after birth: If a child had injuries during or after birth is likely to grow LD. Other causes such as drug or alcohol consumption during pregnancy, physical trauma, poor growth in the uterus, low birth weight, and premature or prolonged labor also result in LD. 
  3. Stress as an infant: If an infant had a stressful incident after birth such as high fever, head injury, or poor nutrition, then the child can grow LD. 
  4. Environment: Excessive exposure to toxins such as lead can be a cause of LD.
  5. Comorbidity: Children with LD have higher chances of attention problems or disruptive behavior problems. 25% of children with reading disabilities also have ADHD. Similarly, 15 to 30% of children with ADHD have a learning disability.

During regular physiological development, children acquire basic sets of basic cognition and motor skills. If the child has any delay or gap in this development then it can be a sign of LD.

Special educator assesses the child’s academic achievements with the help of standard education tests. These tests assess the areas of reading, spelling, written language, and mathematics. The counselor focuses on the behavioral issues that a child might have. When LD is detected, a pediatrician will enquire about the child’s performance in school and provide guidance to parents to get their child’s psycho-educational assessment done. Child psychiatrist checks whether there exist any symptoms of ADHD as there are higher chances of its coexistence. All of these, together contribute to treating children with learning disabilities.

Indiaโ€™s Ministry of Health was established in 1947, after independence. In 1983, the National Health Policy was introduced, which aimed at universal healthcare by 2000. Universal healthcare means that all the people have access to health services, when and where they need it, without facing any hardships or financial obstacles.

SPUTNIK V COVID-19 VACCINE- MANUFACTURING IN KARNATAKA SOON

In the face of a vaccination shortage, Karnataka will house manufacturing plants for Covaxin and Sputnik V COVID-19 vaccines.

While Bharat Biotech’s Covaxin manufacturing facility in Malur, Kolar district, is under construction, Shilpa Medicare has signed a three-year definitive agreement with Dr. Reddy’s Laboratories (DRL) through its wholly-owned subsidiary Shilpa Biologicals Pvt. Ltd. (SBPL), for the production and supply of the Russia-made Sputnik V vaccine from its integrated biologics R&D-cum-manufacturing centre in Dharwad.

By the end of August, the Kolar vaccine manufacturing facility, according to state Health Minister Dr K Sudhakar, will be able to produce four to five crore doses each month.

Sudhakar claimed he attended a video conference with Bharat Biotech founder Dr Krishna Ella, his daughter Dr Jala Ella, and the rest of the team on May 17 in Bengaluru. Sudhakar said, “Dr Ella has assured me that their facility at Malur in Kolar will be able to produce one crore vaccines by June-end. By July-end it will be two to three crores, and their target by August-end is four crore to five crore vaccine doses.”

Dr. Krishna Ella and the directors of Bharat Biotech have also informed him that vaccination doses will be delivered to Karnataka as soon as possible, according to the minister. Sudhakar stated that he had requested a rollout timeline from them.

Meanwhile, SBPL said in a regulatory filing that it expects to produce 50 million doses of the dual vector Sputnik V in the first 12 months of commercial production.

On May 14, DRL soft-launched the imported COVID-19 vaccine Sputnik V. The vaccination is priced at Rs 948 per dosage, plus a 5% GST (retail price of Rs 995.40).

DRL, according to Shilpa Medicare, will help SBPL transfer technologies. SBPL will be in charge of manufacturing, while DRL will be in charge of distributing and promoting the vaccine doses throughout its marketing regions, according to the agreement.

In the near future, Shilpa Medicare stated, the businesses are looking into manufacturing Sputnik Light, a single-dose version of the Sputnik V vaccine.

DR REDDY’S SPUTNIK V VACCINE; TIE-UP WITH APOLLO HOSPITALS

On May 17, Dr. Reddy’s said that 8-9 states have contacted the company about purchasing the Sputnik V COVID-19 vaccine. Andhra Pradesh and Telangana are among the states that have reached out to them, according to the company.

The imported doses of Russia’s Sputnik V COVID-19 vaccine were priced at Rs 948 plus 5% GST, or Rs 995.40 per dose, according to Dr Reddy’s. It stated that the price for both government and private channels will remain the same.

The company plans to partner with hospitals in the metro area that can store vaccines at 18 degrees Celsius.

Last week, Dr Reddy’s received 1.5 lakh Sputnik first dose doses, and the company said it received a second consignment of 60,000 second dose doses over the weekend.

“We have a supply commitment of 36 million doses from RDIF in the next two months,” said M V Ramana, CEO of Branded Formulations at Dr Reddy’s.

Dr. Reddy’s is attempting to obtain further doses from RDIF, according to Raman. RDIF has agreed to supply 250 million Sputnik V first and second doses to the company. Russia will have between 15-20% of the initial supplies.

The vaccine prices will come down once local manufacturers start supplying, which is expected in the next two to three months, according to Dr. Reddy’s.

“They have to absorb the technology from RDIF, have to get approvals from the regulators and then scale up manufacturing,” said Sauri Gudlavalleti- Head of Research and Development. “We are working with the regulator to bring appropriate guidance, on whether the other Indian suppliers with whom RDIF signed supply agreement will have to do clinical trials or not,: Gudlavalleti added.

DCGI has given Hetero, which has a partnership with RDIF, permission to conduct Phase-3 trials of the Sputnik V vaccine.

Gudlavalleti stated that the company plans to approach the Indian drug regulator in the coming weeks to request emergency use authorization for the Sputnik Light single-dose vaccine.

Tie-up with Apollo Hospitals

Dr. Reddy’s Laboratories and Apollo Hospitals declared a collaboration on Monday to conduct a trial launch of the Sputnik V COVID-19 vaccine in India.

Dr. Reddy’s will rely on the Apollo Hospitals network around the country for vaccine storage, transportation, administration, and monitoring.

The Sputnik V vaccine would cost between Rs 1,200 and Rs 1,250 per dose, according to Apollo Hospitals.

According to Dr. Reddy’s, the initial rollout will begin on May 17 in Hyderabad and will be expanded to Visakhapatnam and other metro cities on May 18.

In addition to Apollo, Dr. Reddy’s has a partnership with IHH Healthcare’s Contitental Hospitals.
